The Clinics and hospitals that have a Great Connection license, are permitted to offer sendings to their patients under the terms and conditions of the license. The Clinic ́s need each patient to sign a Patient Disclosure Form and Authorization required by HIPPA. The Clinic/Physician is not the agent, servant, or employee of Great Connection Inc. Great Connection Inc. does not provide, supervise, or control the care that you receive.

The services to patients consist of the one-time wireless transmission to an email address and mobile telephone number that the patients specify, of the images and video clips (collectively, “Images”) that were generated during the screening performed on the Effective Date. When the screening is complete, and in accordance with the Physician/Clinic’s policies and procedures, the Clinic/Physician will cause the machine to send an encrypted copy to the patient. The server will reformat and encrypt the file and provide access to the images through a secure link. The patient can access the images through each link. Once the images have not been accessed for 3 month. The link will no longer be operable.

Therefore the patient should download the images when first using the link. The transmitted images are not part of the patients medical record. Neither the Physician/Clinic nor Great Connection Inc. is responsible for the privacy or security of the transmitted images once the text and email recipients you have designated download the images.

Mobile text messaging and email sending will be used to provide patients and doctors information and images for the patients study.

You will only receive one message per request from 69945 and will not be subscribed to on going alerts. Text HELP to 69945 for help, Text STOP to 69945 cancel. Message and Data Rates May Apply.

The price for the transmission of the ultrasound imaged via email, SMS & MMS is set by the health care provider and is not charged on the mobile phone bill.
